Registration & Immunizations

New to the area or is your child ready to start the educational journey? We’re eager to welcome you!


To register your child at one of our schools, please stop by your neighborhood school to pick up a registration packet or download one below.

*Please keep in mind that you may have to fill out additional documents when you go to your designated school to complete the registration process.

Children who turn five years old before September 1 of each school year may register for kindergarten. We also offer options for early learning. Please contact us at (623) 474.7260 to find out more about our Head Start program.

In addition to completing enrollment forms, you will need the following documentation to enroll your child:

  • Birth certificate
  • Immunization record
  • Proof of residence
  • Transcripts (pupil withdrawal form, grades, etc.)
  • Parent/guardian ID or driver’s license


We require students entering FESD to provide documentation of immunity against certain childhood diseases, with exceptions granted for medical or religious reasons. If your child falls into one of the exception categories, please contact us to complete the correct documentation. Open Enrollment

Arizona state law allows students to apply for admission to any public school, with admittance based on available classroom space. Please complete the form below for consideration.

The deadline to submit the open enrollment application to your preferred school site is May 1. Each school site will provide additional forms to complete and will be included with the open enrollment application.
